4、wo: PTW所有人員必須遵守哈納斯液化天然氣工廠工作許可證制度，應該申請工作許可而沒有申請的, 對責任人罰款200元。在工作許可證發出之前開始工作的，以及在重新生效前進行工作的，都視為無票證作業。發現一次對作業主管罰款200元。All personnel must comply with the Permit to Work system. If a PTW is not applied as required, a fine of 200RMB shall be imposed on the person held responsible.Starting work before the

5、work permit is issued/revalidatedwill be deemed the same as above. If any unsafe tools and equipmentsare found in use,

10、the task supervisor shall be penalized with a fine of 200RMB.承包商工頭應確保本組所有人員都已經經過哈納斯的入場培訓，否則禁止申請任何工作許可。如有違反，對承包商工頭罰款200元。Contractor foreman shall ensure everyone in his group completed the Hanas LNG plant HSE induction. Contractor foreman shall be imposed a fine of 200RMB for the violation. 高風險作業的作業許

11、可可能會要求安全監護人在場。沒有按照作業程序指定安全監護人的，對作業主管罰款200元，指定了安全監護人，而沒有履行安全監護人職責的，對指定的安全監護人罰款200元。High risk jobs may require safety watcher/guard. A fine of 200RMB shall be imposed if task supervisor fail to nominate the safety watcher in accordance with HSE MS. A fine of 200RMB shall be imposed if the nominated sa

A fine of 50RMB shall be imposed for absence without excuse or absence without delegation.所有發出的工作許可應記錄在工作許可記錄簿中，發現記錄不全

17、或者無記錄的，對當值HSE專員罰款50元。All permits issued shall be recorded in the log book. IfLog book not maintained properly, a fine of 50RMB shall be imposed upon duty HSE engineer. 作業結束后，在關閉工作許可前，作業主管或者承包商工頭必須確認工作現場清理干凈，工具設備歸位或者復位。如果違反本規定，對作業主管或者承包商領班罰款200元。Task supervisor and/or contractor foreman shall ensure

A fine of 200RMB shall be imposed upon Task supervisor for violation.作業條件或者作業性質發生改變，應停止工作，重新申請作業許可。未及時停止作業的，如果有安全監護人，對安全監護人罰款200元。沒有安全監護人的對作業主管或者承包商領班各罰款200元，并對屬地負責人罰款100元。Work shall

21、tion manager and maintenance manager.第三章：上鎖掛簽Chapter three: LOTOTO在進行作業之前，必須對將要進行的作業進行辨識，如果存在機器，設備意外的受能、啟動與工藝系統釋放意外危險能量的可能，必須進行能量隔離，通常是通過上鎖掛簽來實現的。未根據本規定進行上鎖掛簽或者其他適宜方式的能量隔離的，對所有當事人罰款200元。 Any energy may be involved during the work must be effectively identified. 24、 200RMB shall be imposed for the violation.在特定情況下，一些特殊隔離點無法做到上鎖，經設備負責人書面同意（在掛簽上背書可接受），可以只做掛簽處理。未經設備負責人書面同意的，對執行人罰款200元。In cases where an isolation point can not be locked-out, written approval(endorsement on tag is deemed as a written approval) must be obtained from equipment responsible person for

A fine of 200RMB shall be imposed for the violation.作業需要兩個以上（含兩個）隔離點上鎖時，必須使用集體鎖。即集體鎖用于能量隔離點，工作人員和隔離執行人的個人鎖用于鎖集體鎖的鑰匙，而不是將個人鎖直接用于隔離點。違反本規定，對隔離執行人罰款200元。A group locking device must be used when a task requires two or more isolation points.When applying the group LOTO, the

26、 group locking devices are applied at the isolation points, and the individual locking devices are used to lock out the key of group locking devices rather than lock the isolation points directly.A fine of 200RMB shall be imposed on the responsible person for the violation. 任何上鎖，包括集體鎖站上的個人鎖，都必須附有掛簽，

27、雖然已經“上鎖”但是沒有掛簽的，或者掛簽填寫不完全的，對上鎖掛簽執行人罰款50元。An information tag must be attached to locking devices in use. If information tag is not in place or the information tag is not filled out correctly, although the lock-out is done, a fine of 50RMB shall be imposed on the responsible person.所有“上鎖”必須有本人解除。如果本人不

28、在廠內，須經過上鎖人書面或者口頭同意并經過上級主管批準，才能解除。違反本規定者，罰款200元。All the lock-outs can only be removed by the person who applied this LOTO. If he is absent from site, the permission from him or the approval from his supervisor must be obtained for removing the lock-out by others. A fine of 200RMB shall be imposed for

29、 violation.任何人不準操作已經上鎖掛簽的設備和系統，除非處于測試的目的。否則罰款200元。Do not operate the equipments being locked out and/or tagged out intentionally unless for testing purpose. 31、sonnel shall test the locked out equipmentsprior to work ensuring the effectiveness of isolation. And then sign the isolation permit. A fine of 200RMB shall be imposed if testing is not done or the isolation permit is not signed 作業需要五個及以上隔離點上鎖時，必須附有PID，PID不可用時需要以草圖示意。違反本規定，對隔離方案編制人罰款100元。PID or a sc

32、hetch(if PID not available) shall be attached if there are more than 5 isolation points in a isolation plan. A fine of 100RMB shall be imposed on isolation planner for violation.當換班時工作尚未完成，設備所屬部門集體鎖、個人鎖及“危險禁止操作”標簽不能被解除，接班人必須先用他的個人鎖鎖上設備所屬單位集體鎖箱并掛簽后，同時在工作票上簽字，交班人才可解除他的個人鎖和簽。未遵守本規定者，對交接班雙方罰款200元。If a w

A fine of 200RMB shall be imposed on both parties if this procedure not followed.第四章：事件事故報告與調查Chapter four: near misses/

34、Incidents reporting and investigation在哈納斯天然氣工廠所有未遂事件及事故都必須報告。如果發現隱瞞行為，針對嚴重程度不同對當事人及目擊者罰款100-2000元。All the incidents and accidents at Hanas LNG plantmust be reported. Depends on the potential severity, a fine of 100-2000EMB shall be imposed for hiding any incidents/accidents.本規定將人員受傷事故分為三個等級，輕微事故（簡單醫

35、療事故，醫療事故），中等事故（單人未導致殘疾的損失工時事故和工作受限事故）和重大事故（多人損失工時事故，單人導致殘疾的損失工時事故，直至死亡事故）。輕微事故如果當事人和責任人及時報告，將不做追究。中度事故經過事故調查，確認責任人后，根據責任劃分分別給予500-1000元罰款。重大事故，經過事故調查，確認責任人后由公司管理層做出最終的處罰決定。對于環境事故和財產損失事故將按照嚴重程度與上述相應等級處理。Injury incidents will be classified into 3 categories for this rule: Minor(FA&MTC), moderate(singl

36、e LTI without disability&RWDC) and Serious(Multi-LTI, Single LTI with disability, fatality and above). No penalty actions shall be taken to against people held responsible if a minor injury has been reported in time. A fine of 500-1000RMB shall be imposed on whom is justified and proved to be held r

37、esponsible for moderate injury incident through official incident investigation. The final penalty decision against person held responsible for serious injury incident shall be made by plant management team after going through the official investigation.The same principle as above applies to environ

42、部門負責協調事故調查。設備事故由維修部門負責事故調查。質量事故由化驗室負責事故調查。人員受傷、安保事故、火災事故和交通事故由HSE部門負責協調事故調查。事故調查協調人須在程序規定的時間內提交事故調查報告。逾期未提交的，對對口部門負責人罰款200元。The investigation of environment damage and production loss shall be facilitated by production department. A fine of 200RMB shall be imposed on the discipline manager for delay of submitting the final report.第五章：安全設備維護與檢查Chapter five: Safety equipments maintenance and inspection設備安全附件必須在第一次啟用前或者停用達到30天，重新啟用前進行測試。違反本條規定，對設備所屬部門負責人罰款200元。Safety systems are to be checked and tes

46、ued each time requiring an override of safety system or equipments. A fine of 50RMB shall be imposed for violation.安全設備無法正常使用時，設備負責人需要立即報告并修理。24小時內不能修復的，需要填寫安全降級登記表。未及時報告者，罰款200元；未填寫安全降級登記表者，罰款50元。Whenever a defect on safety equipments is observed, a defect report shall be issued and the equipments

If repair is not possible within 24 hrs, the situation shall be recorded in defect situation list with associated compensatory measures.If no report made, a fine of 200RMB shall be imposed.
