20、危險源辨識和風(fēng)險評估，有效防范和避免了事故的發(fā)生。經(jīng)過全廠職工的努力，2008年 11 月 27日， 順利通過了安全標(biāo)準(zhǔn)化二級達標(biāo)企業(yè)驗收，并得到考評專家組的一致好評，稱企業(yè)完全有實力通過一級達標(biāo)企業(yè)。（二）加強生產(chǎn)現(xiàn)場安全管理，強化過程監(jiān)控。1 、以生產(chǎn)現(xiàn)場為重點，加強作業(yè)環(huán)節(jié)安全管理，落實風(fēng)險控制措施嚴(yán)。企業(yè)以生產(chǎn)現(xiàn)場為安全工作的重點，加強現(xiàn)場人、物、環(huán)境的管理，認(rèn)真落實危險作業(yè)管理制度，加強關(guān)聯(lián)性作業(yè)協(xié)調(diào)指揮，規(guī)范信息溝通聯(lián)絡(luò)、跨區(qū)域作業(yè)監(jiān)護制度。危險作業(yè)前，必須組織進行危險源辨識和風(fēng)險評價，制定風(fēng)險控制措施，確定作業(yè)方案，經(jīng)審批后實施；作業(yè)過程中若有變更，必須履行變更手續(xù)。嚴(yán)格生產(chǎn)現(xiàn)場

21、動火作業(yè)、光氣管道設(shè)備檢修作業(yè)、進入設(shè)備（有限空間）作業(yè)、高處作業(yè)審批，并隨時進行復(fù)查，保證危險作業(yè)安全可控。為了確保檢修作業(yè)安全，加強項目單位與檢修單位的溝通和協(xié)作，在跨區(qū)域作業(yè)證的基礎(chǔ)上，還必須執(zhí)行檢修作業(yè)條件確認(rèn)和驗收制度，項目單位必須保證檢修項目達到安全環(huán)保要求，具備檢修條件；檢修單位必須保證檢修質(zhì)量達到安全環(huán)保要求，并相互監(jiān)督和確認(rèn)。2、強化對關(guān)鍵裝置重點部位和重大危險源的監(jiān)控和管理。在廠級綜合巡查、專業(yè)巡查和二、三級監(jiān)控網(wǎng)絡(luò)以及視頻監(jiān)控系統(tǒng)的基礎(chǔ)上， 建立完善關(guān)鍵裝置重點部位企業(yè)管理人員定點承包機制，完善應(yīng)急處理方案。（3） 認(rèn)真組織開展安全生產(chǎn)事故隱患排查治理，防范事故發(fā)生。全廠

22、共排查各類問題和事故隱患共298 項，其中廠級（含綜合、專業(yè)及上級檢查）檢查發(fā)現(xiàn)問題和隱患148 項，各二級單位排查出問題和隱患150 項。對檢查中發(fā)現(xiàn)的問題和隱患限期整改，對發(fā)現(xiàn)當(dāng)天就能整改的問題和隱患，均立即整改絕不讓其過夜；對暫時不具備整改條件的，均做到責(zé)任、措施、資金、時限、預(yù)案 “五落實”，制定嚴(yán)密的監(jiān)控措施，杜絕和防范事故發(fā)生。這 298 項問題和隱患已全部按要求整改完成，達到了預(yù)期效果，期間企業(yè)安全生產(chǎn)平穩(wěn)運行。這說明我廠隱患排查治理工作初見成效，真正起到消除隱患、防范事故的作用。（4） 做好安全教育培訓(xùn)，提高全員安全意識和技能。企業(yè)在抓安全教育上，嚴(yán)格執(zhí)行“三級安全教育”，對新

23、進廠的人員按要求進行了廠級教育，同時督促、檢查二、三級安全教育，使他們熟悉、掌握必要的安全技術(shù)知識和自我防護技能，達到要求后方可上崗操作。對換崗、轉(zhuǎn)產(chǎn)、復(fù)工人員，各單位均按要求進行了安全技能和崗位操作法的培訓(xùn)，經(jīng)考核合格后才能上崗作業(yè)。定期組織安全管理人員、特種作業(yè)人員培訓(xùn)取證，建立臺帳，保證全廠安全管理人員、特種作業(yè)人員100有效持證上崗。利用生產(chǎn)間隙，開展了全員安全教育，包括安全知識、安全規(guī)程、崗位操作法、事故案例等內(nèi)容，同時加強對二、三級教育和復(fù)崗、轉(zhuǎn)崗職工的教育，使職工的安全技能和意識不斷提高。（五）開展應(yīng)急救援演練提高響應(yīng)能力。6 月，企業(yè)組織進行重大危險源應(yīng)急救援演練，對各關(guān)鍵裝置
