I -第八條 申報安全質(zhì)量標(biāo)準(zhǔn)化礦業(yè)集團(tuán)公司、縣（區(qū)）等級，必須具備下列條件：1 .考核年度內(nèi)，所屬煤礦均達(dá)標(biāo)，其中一級標(biāo)準(zhǔn)化礦井達(dá)60%及以上；2 .考核年度內(nèi)，礦業(yè)集團(tuán)公司百萬噸死亡率不超過0.5,縣（區(qū)）百萬噸死亡率不超過 1.0;3 .制定并執(zhí)行安全質(zhì)量標(biāo)準(zhǔn)化檢查評比、 培訓(xùn)、考核獎懲制度。第九條檢查考核辦法：1 .安全質(zhì)量標(biāo)準(zhǔn)化煤礦實行分級檢查考評，可采取定期考評 和

36、動態(tài)抽檢相結(jié)合的辦法。省經(jīng)濟(jì)和信息化委員會每半年抽查一 次，抽查比例不低于20% ;市煤礦安全監(jiān)管部門每季度抽查一次， 抽查比例不低于50%;礦業(yè)集團(tuán)公司每季度普查一次；煤礦每月進(jìn)行一次全面檢查。每次檢查的記錄均要存檔， 作為月度、季度、 半年和全年考核的依據(jù)。2 .各單位于次年1月份將本單位上一年度安全質(zhì)量標(biāo)準(zhǔn)化建 設(shè)檢查評比結(jié)果和申報等級及本年度安全質(zhì)量標(biāo)準(zhǔn)化建設(shè)規(guī)劃 上報省經(jīng)濟(jì)和信息化委員會，凡不按規(guī)定上報的，取消其等級評 定資格。3 .每年2月底前對申報單位進(jìn)行抽檢、復(fù)核、評定、評比和 命名。4 .已達(dá)標(biāo)的單位次年依復(fù)查得分決定升、降級；復(fù)查不能鞏 固的，撤銷安全質(zhì)量標(biāo)準(zhǔn)化單位稱號。-

46、量標(biāo)準(zhǔn)化煤礦由各礦業(yè)集團(tuán)公司或產(chǎn)煤市 煤炭管理部門向省經(jīng)濟(jì)和信息化委員會申報，省經(jīng)濟(jì)和信息化委員會組織抽查初評，報國家煤礦安監(jiān)局評定。3 .安全質(zhì)量標(biāo)準(zhǔn)化礦業(yè)集團(tuán)公司、縣（區(qū)）由礦業(yè)集團(tuán)公司 或產(chǎn)煤市向省經(jīng)濟(jì)和信息化委員會申報，省經(jīng)濟(jì)和信息化委員會組織抽查評定。第十一條對達(dá)到安全質(zhì)量標(biāo)準(zhǔn)化礦井以及質(zhì)量標(biāo)準(zhǔn)化礦業(yè) 集團(tuán)公司、縣（區(qū)）進(jìn)行獎勵，其獎勵辦法按山西省煤礦安全 質(zhì)量標(biāo)準(zhǔn)化建設(shè)實施意見規(guī)定執(zhí)行。其中，達(dá)到一級標(biāo)準(zhǔn)的， 30萬噸/年及以上煤礦主要負(fù)責(zé)人獎勵1萬元，其他煤礦獎勵5000元；達(dá)到國家級標(biāo)準(zhǔn)的，30萬噸/年及以上煤礦主要負(fù)責(zé)人 獎勵2萬元，其他煤礦獎勵1萬元；連續(xù)2年達(dá)到國家級標(biāo)準(zhǔn)
